Required documents issued in Poland

Candidates with 'QUALIFIED' or 'RESERVE' status who have received their education in Poland must deliver the original paper documents listed below within 4 days after the announcement of the list of candidates to the Admission Committee at the relevant Faculty.

 

Candidates for bachelor (first cycle) or 5 years master (long-cycle) studies

  1. a self-signed PERSONAL QUESTIONNAIRE, generated and printed from the IRK system;
  2. an up-to-date COLOUR PHOTOGRAPH measuring 35 mm x 45 mm (ID or passport size);
  3. original, certified or notarised copy of HIGH SCHOOL DIPLOMA together with Polish baccalaureate exam (MATURA);
  4. Non-EU, non-EFTA, non-OECD citizens: one of the documents listed below (original or notarised copy): 
  1. passport
  2. Card of Pole (Karta Polaka)
  3. permanent residence card + decision on granting permanent residence permit in Poland
  4. formal decision on Polish origin
  5. formal decision on refugee status granted by the Republic of Poland
  6. formal decision on temporary protection in the territory of Poland
  7. formal decision on subsidiary protection in the territory of Poland
  8. long-term residence permit of the EU
  9. document confirming the type and degree of kinship with Polish citizen (spouse/ascendant/descendant)
  10. Polish language certificate issued by PKPZJP (min. C1 level)
  11. formal decision in temporary residence permit on the basis of art. 159 sec.1 or art. 186 sec. 1 pt. 3 or 4 of the Act of 12 December 2013  on foreigners along with temporary residence card.

    6. EU, EFTA, OECD citizens: passport (original or notarised copy).

    7. LEGAL REPRESENTATIVE STATEMENT OF CONSENT - candidates under 18 

 

Candidates for master (second cycle) studies

  1. a self-signed PERSONAL QUESTIONNAIRE, generated and printed from the IRK system;
  2. an up-to-date COLOUR PHOTOGRAPH measuring 35 mm x 45 mm (ID or passport size);
  3. original, certified or notarised copy of BACHELOR or MASTER DIPLOMA;
  4. SUPPLEMENT/TRANSCRIPT OF RECORDS and list of subjects;
  5. Non-EU, non-EFTA, non-OECD citizens: one of the documents listed below (original or notarised copy): 
  1. passport
  2. Card of Pole (Karta Polaka)
  3. permanent residence card + decision on granting permanent residence permit in Poland
  4. formal decision on Polish origin
  5. formal decision on refugee status granted by the Republic of Poland
  6. formal decision on temporary protection in the territory of Poland
  7. formal decision on subsidiary protection in the territory of Poland
  8. long-term residence permit of the EU
  9. document confirming the type and degree of kinship with Polish citizen (spouse/ascendant/descendant)

    7. EU, EFTA, OECD citizens:  passport (original or notarised copy);

    8. NAWA DECISION granting the scholarship (NAWA scholars only) and photocopy of passport.

IMPORTANT

  • Failure to submit paper documents by any of the above deadlines will result in non-enrolment on the student list;
  • A candidate not qualified for admission is not entitled to submit documents;
  • Documents may be submitted in person, by a third party, or sent by post or courier (the date of delivery, not the date of posting, is what counts). We do not require authorisation for submission of documents by a third party;
  • The submission of documents by persons with "reserve" status does not mean being admitted. A "reserve" candidate takes the place of a "qualified" person (according to the ranking) in the event that this person has not provided a complete set of documents by the set deadline, has resigned from the recruitment process or has been disqualified.
